If you are providing additional phone numbers or email addresses, please use the "Comment" area and note to whom each item belongs (whose email address) and description of phone (home phone, husband's cell, wife's cell). 

If you are providing a temporary address (winter address), you MUST provide the dates in which each of your addresses should be active.
